Please note this is only the registration part of your claim/complaint, therefore we would only need some basic information. Your claim package will be sent via email within the next 24 business hours of registering your claim to the email that you provide to us.

 

There are procedures set by the government that you as the customer, and your moving company, are obligated to follow. Please read the back of the moving contract, labeled “BILL OF LADING”. You will find the various terms and conditions governing your type of moving contract on the reverse side.

Note: Once you fill out the requested fields below you will receive your claim package via EMAIL within the next 24 HOURS. Please check your junk or spam mail folder as well.

Once the new claim applications form is submitted please send us the following documentation to cs@cafcps.org :

- Pictures (for damaged items only individually labeled)

- Contract (also known as Bill Of Lading) 

- Inventory List (Long distance moves only)
